Independent Study/Internship Form - Form is for access to Directed Study, Masters or Doctoral Research, and Thesis and Dissertation courses, and Internship. Form is to be signed by faculty instructor of the independent study course or Graduate Coordinator, for off- campus internships. Students with off-campus internships and F1 visa must register for internship course, CSCI 7007 3 credit hours under his/her major professor CRN. MS Core Competency Form (Thesis only) MS Core Competency Form (Non-thesis only) MS Cybersecurity and Privacy Core Competency Form ( Thesis only) MS Cybersecurity and Privacy Core Competency Form ( Non-thesis only) Phd Core Competency Form Foreign Travel Assistance Program CSCI 7200 MS Project Form - Effective, beginning Spring 2023.This form is required for all School of Computing students who are taking CSCI 7200, in the second/final semester of CSCI 7200, and must be completed by Reading Day in second semester. CSCI 7200 requires total minimum 4 credit hours. Two project professors required on form. The MS Project form will be uploaded to elC. The MS Project Final Report is to be emailed to Graduate Coordinators prior to Reading Day. Late Add Form- Use this form to late-add a CSCI course after the end of drop/add period. Late Adds are permitted within the first 4 weeks of the semester. After this time, no Late Add will be processed by School of Computing. Email form to instructor of course , and then to Dr. Thiab Taha. Interim Director School of Computing,email@example.com. Student is responsible for submitting form to Registrars office. (There must be seats in the course!) ( please be sure to check the impact of late-adding and withdrawing from a course with Student Accounts at firstname.lastname@example.org). Request for CSCI Domestic or International Travel Funds-This request is for travel funding for students presenting their research at conferences and professional meetings. CS graduate student must have abstract, acceptance (if available), and request Graduate School funding prior to making a request for CS departmental funding. See CS listserv emails for deadlines. Franklin College Business Services Center Travel Authorization Form ( email to email@example.com) Travel at UGA- please read NOTE: For virtual conference, the student DOES not need a Travel Authorization. Students are responsible for finally submitting the travel expense report.
